ATHLETES
All athletes must have a current Special Olympics Athlete Application – Medical Consent Form on file. In 2024, Special Olympics Minnesota changed the rules regarding the application/medical form so that now the form is submitted annually and a physical is no longer required. The new rules became effective August 15, 2024.
Determining if your form is current
- Forms that were submitted prior to August 15th, 2024, are valid for 3 years.
- Forms submitted after August 15th, 2024, need to be completed annually.

Apply and renew
To apply to be a new Special Olympics athlete or to continue your SOMN athlete status, please submit a copy of the application/medical form to the RAVE:

The RAVE will forward a copy of your application/medical form Special Olympics Minnesota.
